Procedure
Logon to Sap Solution Manager and select: Service Operations –> Document Template Designer. Click in New
Select The File Type and define all the fields (Remember to define the WebService name that you create before this document) and the Object Type BUS2000116 (CRM Activity).
IMPORTANT: DONT FLAG “Make Interactive”, This topic will be covered in a future blog
After Press the button “Start Designer”, Solution Manager will be open the Adobe Livecycle Designer, and will be possible to press the Download Button, press the button to download the Template file to your computer.
When you open the file with the Adobe LiveCycle Designer, you will be receive the following Information, (This is the XML Schema used but the form to capture the information)
Now you will see the SAP Field defined in the WebServices in the “Data View” Tab.
You can start to design your Form using Drag&Drop
Now is time to upload the template to Sap Solution Manager again, you need to go back to Solman and browse your file, then click in Upload.
HOW-TO USE THE FORMS
Create a new Attachment in any Incident or Change Document, Open the Attachments Tab and Click in “With Template” and select the right one, the system add the file selected like attachment, If you click on the file, Sap Solution Manager will be open the PDF file with all the information related to your ticket.

I learned a lesson: While cleaning house in “My Documents”, I didn’t know what the folder “My Fragments” was for so I deleted it.
Turns out it is required by Adobe Livecycle Designer 8.1, which SAP licensed and is one of the optional developer tools for SAPGUI and Netweaver Developer Studio. Deleting “My Fragments” caused my SAPGUI to lock up whenever I tried to open an Adobe Form. When I clicked in it, a SAPGUI popup said “Server Busy: This action cannot be completed because the other program is busy. Choose ‘Switch To’ to activate the busy program and correct the problem.” Neither ‘Switch To’ or ‘Retry’ got me out of the loop – I had to log in again and kill my first session.
Executing C:Program FilesAdobeDesigner 8.1FormDesigner.exe directly showed that it could not open …My DocumentsMy Fragments.
Fragments are new (“preview”) in 8.1 and are not supported by SAP.
Uninstalling and reinstalling did not fix it. I had to:
Create a new “My Fragments” folder
Execute C:Program FilesAdobeDesigner 8.1FormDesigner.exe
Go to menu option Edit–>Fragments–>Create Fragment…
Create new fragment library …My DocumentsMy FragmentsFragment1.xdp
Searching Adobe’s site, I found this note: http://kb2.adobe.com/cps/403/kb403720.html. That suggests it may also work to move folder C:Documents and Settings{user}Application DataAdobeDesigner to another location (like C:Temp,) which would force Designer to re-initialize all its settings. I haven’t tried that yet.
